Preparing for your Stay

For all patients

Please remember to bring:

  • Insurance information
  • Medical record, prescriptions, general practitioner contact information
  • Contact information for loved ones
  • Paperwork and contact information for healthcare power of attorney or medical decision-maker
  • Change of clothes for discharge
  • Additional advanced directives

For parents

  • Child's pajamas, favorite blanket or toy or other item to make your child more at ease and comfortable with his/her surroundings.
  • Special adaptive equipment or any equipment used for special care needs at home such as wheelchairs for chronic care, strollers, splints, etc.
  • If planning to stay overnight, please bring personal items such as medications, clothes, toiletries, phone chargers.
  • Nursing mothers, please bring your breast pump and supplies in case you need it.
  • Every discharged child under 8 years old or 80 lbs will need to go home in an approved restraint (car seat or booster)
