UNC Health Chatham’s Post-Acute Care Unit helps you heal after a hospital stay — like after a stroke or surgery — so you can safely get back home. We provide rehabilitation services and regular physician visits that allow you to regain your strength and recover with the support of our dedicated care team.
Our post-acute care program provides:

Regular weekly visits from a physician to manage your continued care, with more frequent physician visits when needed.

A care team that includes physicians, nurses, care managers, pharmacists, dietitians and therapists, who work together with you and your loved ones to discuss your progress, goals and discharge plans.

Physical, occupational, respiratory and speech therapy to help you move better and take care of yourself safely when you go home.

Skilled nursing care and treatments, including IV antibiotic therapy and wound care.
Post-Acute Care Program Advantages
Our post-acute care program helps you heal after a hospital stay, like after surgery or a serious illness. It offers several advantages over a traditional nursing home, including regular doctor visits and personalized rehab. Other benefits include:
- UNC Health physicians are available so you can get medical help anytime you need it
- More nurses available, so you get the care and attention you need, when you need it
- Immediate access to diagnostic services, including labs, such as blood draws, and imaging, such as X-rays
- Private patient room with a television, a telephone and wireless internet right in the medical-surgical unit of the hospital
- Less chance of being readmitted to a hospital
- Interpreter services
