Diabetes Self-Management Program

Diabetes affects millions of Americans and when left untreated can lead to heart disease and stroke, damage kidneys and nerves, and affect eyesight, circulation and healing capacity.

Chatham Hospital’s Diabetes Self-Management Program can help you learn to control the disease and to lead a healthier, happier life. The program is staffed by a team that includes two nurses (one of whom is bilingual), a pharmacist, a dietician, a bilingual nutritional counselor, bilingual lay health advisors and two consulting physicians. They use a combination of individual assessments and counseling, home visits and group seminars to cover topics that range from nutrition and meal planning to the prevention, detection and treatment of complications.

The Chatham program has been awarded a prestigious Recognition Certificate by the American Diabetes Association (ADA), indicating it complies with the ADA’s national standards and offers the top-quality education essential to effective diabetes treatment.

To participate, consult your personal physician for a referral or call 919-663-2113 (English-language class) or 919-663-9165 (Spanish-language class).

Chatham Hospital and our Emergency Services department will move into our new facility, in Siler City's Central Carolina Business Campus near the intersection of US 64 and US 421, on July 28 at 7 a.m. All inpatient transfers to the new facility will also begin at that time.
